W tej przygodzie bohater próbuje odeprzeć atak armii Mentala (Tah-Um), znanego z poprzednich części, który chce przejąć władzę nad Ziemią. Walkę toczymy na tle walących się starożytnych świątyń oraz miast futurystycznego Egiptu. Twórcy w dalszym ciągu stawiają na beztroską, zręcznościową i krwawą rozgrywkę oraz dużą porcję specyficznego, nieco prostackiego humoru. Dzięki temu dostajemy dość klasyczną strzelankę pierwszoosobową z kilkoma nowymi pomysłami.

Cechą charakterystyczną serii jest to, że walczymy z prawdziwymi hordami wrogów. Nie zmieniło się to w Serious Sam 3 i ponownie przychodzi nam stoczyć bitwę z dziesiątkami zróżnicowanych przeciwników jednocześnie. Większość potyczek rozgrywa się na dość obszernych mapach, więc zawsze mamy spore pole manewrowania. Producent szczyci się tym, że nie zawarł w grze modnego systemu chowania się za przeszkodami, czy możliwości eliminowania przeciwników z ukrycia. Zamiast tego dostajemy porcję czystej akcji. W grze nie mogło zabraknąć kilku nowości. Serious Sam dysponuje teraz potężnymi atakami wręcz, które pozwalają wyeliminować wrogów z bliska. Do naszej dyspozycji oddano też pokaźny zestaw wyszukanego sprzętu, czyli tak lubiane, zwariowane bronie. Trzecia odsłona serii nie byłaby kompletna bez specyficznych przeciwników – ponownie zmagamy się z takimi istotami jak bezgłowe kamikaze, jednookie monstra, czy rozpędzone byki. Na naszej drodze spotykamy wiele innych szkarad, w tym ogromnych bossów.
